with Dr. Capper

Please provide the following information and Dr. Capper will contact you to schedule a meet and greet.

Established Patient Communication NON-URGENT

Elation Passport is the preferred method of communication. This is for a few reasons: 

 

  1. It ensures that your personal health information is protected

  2. It creates a written record of our communications

  3. It allows me to send you communications without being disruptive so that you may review them at a time that is convenient for you. 

Urgent, Life-Threatening, and Emergency Situations

Dr. Benjamin Capper and Capper Medical Corp does NOT offer urgent care, emergency care, or hospitalization services, and does NOT have providers on-call. 

 

For URGENT MEDICAL CONCERNS, go to a local urgent care. I will provide you with a list of preferred partners upon request. 

 

For LIFE THREATENING AND EMERGENCY SITUATIONS, go to your nearest emergency department or call 911. If you require hospitalization, you will be cared for by a hospital medicine physician and the on-call hospital consultants.

 

For MENTAL HEALTH CRISIS: If you are contemplating suicide or experiencing another mental health crisis, please use the following resources:

1) Call 911 if there is an immediate threat to the life of you or someone else.

2) Call 9-8-8 to reach the Suicide and Crisis Lifeline

3) Call 1-866-998-2243 to reach the Ventura County Crisis Team
