Insurance & Payment Policy

Medicare

Dr. Capper is in-network with Part B Medicare. Under this arrangement, Medicare determines the cost of each service. Medicare pays 80% of the bill. If you have a secondary Medicare supplemental plan, they will pay the remaining 20%. If you do not, you will pay the remaining 20% at the start of the visit. Dr. Capper is not "in-network" with Part C Medicare (also called Medicare Advantage) because under that arrangement, Medicare pays a private insurance company to administer your benefits.

Private Insurance

Dr. Capper is not in-network with any private insurance plan. If your private insurance plan has out-of-network benefits, you will pay Dr. Capper the full amount at the start of the visit. Dr. Capper will then submit the bill to your insurer and they will reimburse you according to your out-of-network benefits agreement.

Direct Payment

Patients who do not have Part B Medicare have the option to pay Dr. Capper directly for medical services. The fee encompasses the care provided before, during, and after the visit.

[2] New Patient Visit (60 min) - $300

  • Scheduled after Meet and Greet

[3] For Established Patients:

  • Core Visit (40 min) - $250 (book online)

  • Focused Visit (20 min) - $150 (booked by Dr. Capper)

Method of Payment

  • Payment is collected at the start of the visit.

  • Each visit charge pays for the work performed before, during, and after the visit. 

  • You may use FSA and HSA funds.

  • You may also pay using cash, check,debit/ credit card, bank transfer, Zelle, and Apple Pay.
